Swan Ganz Pulmonary Catheter Placement Video
This is a pulmonary artery catheter with the tip shown. We're now testing the balloon at the tip. The sheath will now be applied to the tip with the gray end being inserted first and pushed through the other side; this is pulled all the way to the 100 centimeter mark which is designated by the two wide black lines.
The gray end is screwed in place to secure the sheath such that it doesn't slide back and forth on the catheter. The blue end is then pulled and drawn back to approximately the 50 centimeter mark designated by the single wide black line.
This is the cordis catheter hub which was has been sterilely draped. It is in the right subclavian vein position. We are now flushing all the ports, the PA port, the CVP port, and the Med port. The catheter is now being inserted with the curve directed inferiorly. It will be advanced to the 15 centimeter mark which is shown right there.
The balloon is then inflated and this is the CVP wave form. The catheter is now being advanced. And we have a right ventricular wave form as displayed there. The catheter is now advanced fairly quickly through the right ventricle until we get to the pulmonary artery wave form, which is right there, and now it is advanced slowly until you get to the wedge, which is right there. The wedge, the balloon is now deflated and then you get the pulmonary artery wave form and then inflate it, and you have the pulmonary capillary wave form right there.
And there is the pulmonary artery wave form again. And we have the inserted the catheter to about 45 centimeters. We're now going to close the system by securing the sheath to the cordis hub, there is a white knob on the hub as is designated right there and a little notch in the blue sheath that secures right into that notch.
There is the completely closed sterile system; there is a tube that extends from the cordis catheter that can be used for medication infusions or for blood draws. This can also be used for pressers.
And an op site sterile dressing is finally applied, and a chest x-ray will be obtained for pulmonary artery catheter tip placement.
